AI Gateway adds confidence-based decision fallbacks

AI Gateway now supports confidence-based fallbacks, allowing escalation to a secondary model when the primary model's confidence in its answer falls below a set threshold. This feature applies to Choice, Score, and Boolean questions and can combine multiple conditions for escalation.
